There are so many conditions of the breast that go poorly understood because of the emphasis on detecting breast cancer early. While finding breast cancer early is indeed of utmost importance, this column is devoted to the other conditions that are significantly encountered in the breast.
Breast size: There is often some difference in the size of a woman’s breast. This is often magnified because of a woman’s concern that this means something is wrong or that she looks “funny”. Even though there are rare cases of women with only one breast developing, most commonly there is only a slight variation in the size, usually only noticed by the woman herself. This is benign when it has “always been that way” and unless it gets to be a psychological barrier for the woman, does not need to be addressed. Plastic surgery is the answer when the difference is significant for the woman.
Breasts that are too large is another problem that sometimes is significant. Some women are just genetically programmed to have breasts that are painfully large. This tendency is not dependent on being “fat” but carrying extra weight can aggravate the condition. Women can be miserable. It can affect a woman’s choice of activities, causing women to avoid playing tennis, jogging, or other vigorous activities. Some women develop deep tender grooves over their shoulders from bra straps digging in and/or they suffer from chronic neck and upper back pain that can cause daily misery. It is a woman’s judgement call to decide when this condition needs to be addressed surgically. Many of my patients are grateful to have had the surgery and never look back. However, scarring can be a problem especially in keloid-formers and successful breast feeding can be made impossible. It is a big decision.
Breast lumps: Benign lumps can be divided into solid or cystic lesions. Many breast cysts that I see in my practice are discovered by the patient. They appear all of a sudden resulting in a visually obvious, tender lump. We usually go ahead with draining the cyst in the office. If it goes away completely with draining, the fluid is clear to milky, and the cyst does not recur, nothing further needs to be done. Some women are prone to this problem and are very familiar with this plan. Other cysts are not noted until a mammogram is done. Often, an area of density on the mammogram will turn out to be a cyst or simple fluid collection when examined by ultrasound. As long as all the cyst criteria are benign, i.e. thin-wall, single cavity, clear, etc., nothing further needs to be done. Anytime the cyst is complicated in appearance, recurrent, or filled with bloody fluid, biopsy should be considered.
The most common solid lump in the breast is something called the fibroadenoma. These can be seen even in the teenage years. They have specific criteria associated with them and sometimes are just observed over time. I lean towards recommending removal for lesions of any size for several reasons. First, patients worry if we are wrong and there is something in their body that will “get them”. Second, these lesions can grow and the bigger it is when removed, the larger the scar or breast defect may be. Third, follow-up can be difficult because of distortion from the lesion.
After a blow to the breast, especially after something like a shelf or table corner hit, or a car accident with seat belt bruising, the breast can undergo fat necrosis in that area, forming a firm nodule that eventually softens but may lead to some long-term distortion.
Nipple discharge: This is very common. When it comes from both nipples, is milky, and is not associated with pregnancy, it can represent an abnormal increase in Prolactin hormone and should be evaluated. Bizarre nipple discharge that is green or black is associated with fibrocystic change in the breast and usually only comes from one nipple at the time. Despite the strange appearance, this is a non-worrisome process. Bloody nipple discharge can be associated with a benign growth in a milk duct called a papilloma. Still, any bloody drainage should be further evaluated to exclude a cancer as the cause.
Commonly, many of the above symptoms are seen in women with fibrocystic change. Up to 40% of women have this condition that can promote the development of cysts, densities that can make the breast feel like beanbags, and colorful nipple discharge. Forty percent, that is a lot of women. All of the conditions mentioned here are very common. Benign changes including swelling before a period or any of the changes mentioned above will often be encountered by a woman. The above descriptions are provided to let women know that most changes don’t represent cancer. One should not be afraid! It is certainly prudent however to see your doctor about any change you notice that is different for you. Particularly, bloody drainage, lumps, or thickened areas should prompt a visit. Let your physician help to determine what evaluation should be done to help you rest assured.

Pre-planning with your surgeon will go a long way toward ensuring that when your breast augmentation experience is complete, you'll be happy with the results and feel very comfortable with your appearance.
First, it is important to choose an experienced, board-certified plastic surgeon specializing in breast augmentation to help you decide how much larger you want to be than your current breast size. A boarded certified plastic surgeon can evaluate your anatomy and take accurate measurements of your chest and breasts to determine the proper proportions for your frame as well as take into consideration how loose your skin is. You must have enough tissue to cover the breast implants for an aesthetically pleasing augmentation.
Most women want to make sure they will look natural, but improved. It is my opinion that the larger you go, the more you risk an unnatural look. Also, larger implants are often too wide for a smaller body. I feel that it is better to have as accurate a prediction process ahead of time as possible. Implants that are too large not only will upset the aesthetic balance of your figure, they also may be more prone to sagging and ultimately give you a “matronly” look. The American Society for Aesthetic Plastic Surgery (ASAPS) offers helpful tips to women trying to make this decision with which I heartily agree with and pass along to my patients: Communication is key! Be frank and open in discussing size. Select several photographs from magazines illustrating your ideal proportions, and bring these to your consultation. Be as specific as possible about what you like about them, but realize that your own "after" pictures will reflect your desires and who you are. Bring examples of individuals with a similar body type to yours. Don’t define your desired size only in terms of bra-cup size. Cup size can vary significantly among bra styles and manufacturers. Maintain realistic objectives. The best approach is to select implants that will look like a natural part of your body. Ask to see before & after pictures, not only to judge your surgeon’s work, but, to get an idea of what size implant you will need. Obviously, the breast size you start with will influence the implant size you choose. If you start as an A cup, you'll need more volume to take you to a C cup than a woman who starts as a B cup. I have actual breast implants that you can try on during your visit, in addition to using the 3D imager, if you so choose. It's important to note that volume refers to size, not breast shape. Breasts that have sagged over time may need a breast lift in addition to augmentation to achieve the look you want.
After you've presented your goals, listen to what your surgeon has to say. If my professional and experienced opinion is that your selection is too big for your frame, I hope you will listen to why I may feel that way. Splurge for one (and make sure to get the free fitting to make sure you're getting the right size!) and treat yourself to a nice bra on the days you're wearing something a little more revealing than normal!
Don't be worried if you're wearing a swimsuit--you can purchase swimsuits that have padding or under wire to help you get the look you want even when you're wearing less than normal!
Use makeup. You know that powder foundation that was a shade too dark for your face? Use it on your chest! No joke! Using a blush brush, brush a little bit of powder foundation on your cleavage, creating a v-shape (go a little on top of your breast to create a natural line). This can help enhance your cleavage and, even if you don't have it, allows you to flaunt it!
Wear Stripes – Horizontal stripes will enhance the image of your breasts, making them appear to be a lot larger than they actually are. You’ll be surprised at how much bigger they will appear just from wearing some stripy clothes.
Bright colors and ornaments will attract more attention to your breast area and make your bust look bigger. The same is true with ruffles and glitter.
Use accessories and small details to visually enhance the cleavage area. A thin chain with a small pendant and delicate design will make your breasts look bigger by contrast.
Wear tight and thick materials that will add some volume to your bust.
Make sure you have your back straight. This pushes your breasts out and gives them better shape.

Breasts are important to both men and women. Women's self-confidence is often tied to the size of their breasts, but not every woman ends up with the kind of breasts that she is happy with. Though many women resort to expensive and risky surgery to enlarge their breasts, there is a simpler, more natural method which costs nothing. If you massage your breasts for 20-30 minutes a day, they should grow one cup size larger in 30 days.
Massage does two things to stimulate growth. One benefit of massage is increased circulation. Since phytoestrogens travel through the bloodstream, the more blood you have flowing to your breasts, the easier it is for your breast receptors to pick up what they need in order to grow. Another benefit of massage is prolactin production. This important breast-enlarging hormone is triggered by regular stimulation of the breasts and nipples.
To perform the massage, start by rubbing your hands together as fast as you can to generate heat and energy. Once your hands are warm, place them on your breasts. Rub inwards with your hands and continue around your breasts in a circle. Your right hand will circle in a clockwise direction, and your left hand will circle counter-clockwise (as you look down). Do a minimum of 100-300 circular rubs in the morning and another 100-300 circular rubs before you go to bed. Each circular rub should last about 2 seconds. Pause occasionally and rub your hands together to re-warm them. The full 300 rubs should take about 10-15 minutes. By doing this twice a day, you should be able to increase a cup size within 30 days.
If you want to use a massage oil, the following mixture has been reported to produce great results in increasing the size of the breasts. Mix 9 drops of geranium oil with 16 drops of Ylang Ylang oil in a base of 50 ml of almond oil. Rub this into your breasts twice a day either during your massage or separately.
A good side effect to doing this is that your menstrual flow will be reduced dramatically since the breast massage stimulation will create a state within your body where the blood you normally lose through your period will rush up to enlarge your breasts. Another good side effect is that the surge in hormones will lessen your PMS. You will also experience increased nipple sensitivity, but avoid touching your nipples when doing your breast massage.

Breast growth isn’t just a literal term. You can actually make your breasts appear to be bigger by performing an exercise that “lifts” them.
Underneath your breasts are a series of muscles called the “pectorals”. These are what you can see on men’s chests and are also what can lift up the breasts and make them appear to be more round and perky.
There’s a very good exercise which works these muscles and makes your breasts seem a lot bigger:
- Lie down on a mat. Hold one weight in each hand and extended your arms out at shoulder level. Raise both arms straight up together above your body, keeping your elbow slightly bent, so that the weights meet over your chest. Return the weighs out to your sides at shoulder height. Repeat the exercise 13 to 15 times.
- Lie down on a mat. Extend your arms and hold the weights up in the air over your chest. Bend your elbows and lower the weights toward your chest, with your elbows out to the sides at shoulder level. Extend your arms straight back up over your chest. Repeat the exercise 13 to 15 times.
- Take a weight in each hand, extend your arms out to each side and do 15 small, backward circles about a foot in diameter. Widen the circles slightly and do another 15, then repeat these two steps going forward.

Breast development begins at puberty and is regulated by the balance of specific hormones. Saline breast implants are implants with a silicone casing that is filled with saline (salt water). They are heavier than cohesive gel implants and tend to feel firmer and different to the breast tissue. It is therefore harder to sleep on your front and they often feel cold.
Saline implants have a very high incidence of rippling, which can be visible. This results in a very unsightly appearance of a wrinkled breast. Try holding a balloon filled with water by your fingers and you can see a similar effect.
There is also a very high chance for saline implants to deflate suddenly. When this happens on one side, you end up with lopsided breasts. There is an urgency to replace the implants with another operation.

The surface of the breast implants can either be smooth or rough (textured).
Textured surface implants have a slightly lower incidence of capsular contracture (thick scarring around the implant) compared to the smooth surfaced implants. However, there is a higher incidence of rippling of the breast when textured implants are used.
Nowadays, the risk of surgery to correct capsular contracture is about 5%. This is a significant decrease compared to many years ago when liquid silicone gel implants were used. Surgery for capsular contracture may not necessarily prevent it from happening again. Nowadays, when capsular contracture is first detected, medications are first used to try and soften the hardening. If this is not successful, then surgery can be considered.
Currently, the silicone casings are made much stronger and thicker than the original silicone implants from years ago. This is to lessen the chances of gel leakage, implant rupture and capsular contracture.
The cohesive silicone gel inside the casing is not like a liquid silicone from years ago. It has the consistency of ‘Turkish Delight’ and feels more natural like breast tissue. When the implant is sliced in half for demonstration, the cohesive gel tends to stick together and does not flow like the liquid silicone.
Generally, the gel in the tear drop implant is of a firmer consistency because the shape has to be maintained. The gel in the round implants is generally softer, so that in an upright position the gel falls downwards to try and form a tear drop shape.

The selection of the breast implant to be used is dependent on the dimensions of your breasts, the shape of your breasts, your skin elasticity and thickness as well as your expectations of the breast shape you want.
For example, if you want to have a fuller upper half of your breast, then a high profile round implant is generally used. If you have more breast tissue in your upper half of your breast with a very short distance between your nipple and your lower breast crease, then you may require a tear drop implant. This is to expand the lower half of your breast in order to achieve a more aesthetically pleasing look.

The shape of the breast implant is either round or tear drop, with different base widths and projections.
Round implants have a round base. In round implants with the same volume, the width of the base determines the projection (profile) of the implant. The narrower the base is, the higher the projection (profile of the implant).
Tear drop (anatomical) implants look like a tear drop when viewed from the side.
The upper half of the implant is flatter and the lower half is fuller. Tear drop implants come with different bases and heights, which affect their projection (profile). The surface of all tear drop implants are textured. There are markers on the surface of the tear drop implants to guide the surgeon to align the implant in a correct position.
After breast enlargement, there is always a risk of the breast implant rotating (spinning) in its pocket (space). When a round implant rotates, the shape of the breast is still the same. However, when the tear drop implant rotates, the shape of the breast will become distorted. There is the very small risk of this happening. The textured surface of the tear drop implant, use of drains and wearing of stabilizers after the operation helps prevent this rotation from happening.
Tear drop implants also have a slightly lower incidence of capsular contracture (thick scarring around the implant). This is because of its shape and its textured surface.

Premature start is a condition in which the breasts of baby girls begin to enlarge. It is usually a temporary, harmless condition. Breast enlargement in infants and young girls is sometimes the first sign of early (precocious) puberty, but this is uncommon.
What is premature start?
Premature start is enlargement of the breasts in infant girls. Most often, breast enlargement is the only abnormality. It is occasionally the first sign of early (precocious) puberty. This is more likely if the breasts become enlarged after ages 2 to 3.
Usually there is no apparent cause of early breast enlargement, although it can result from exposure to medications or to sources of the hormone estrogen. The breasts may remain enlarged for as long as a few years but eventually go down in size before your daughter starts puberty.
What does it look like?
- Your daughter’s breasts start getting bigger.
- One or both breasts may be enlarged. They may go up and down in size.
- Breast enlargement usually occurs before age 2. Occasionally, a baby girl is born with enlarged breasts.
- There are no other signs of puberty, for example, growth of pubic hair (hair around the genitals) or rapid body growth.
- Medical tests, if performed, show no other signs of approaching puberty.
- Breasts eventually stop growing and may become reduced in size. It may take a few years before the breasts completely return to normal. Girls go on to have a normal puberty.
What causes premature start?
- Usually, no specific cause is identified.
- Premature breast enlargement can be caused by exposure to the hormone estrogen, for example, a child eating the mother’s birth control pills.
What are some possible complications of premature start?
- Usually none. The condition often goes away on its own, although this may take a few years.
- Infrequently, premature start is the first sign of early (precocious) puberty. This is most likely when the breasts start to enlarge after ages 2 to 3, accompanied by other signs of puberty such as an enlarged clitoris or development of pubic hair. Treatment may be needed to halt the process of early maturation.

How is premature start treated?
- Usually, no treatment is needed. Your daughter’s breasts will eventually go down in size or stop enlarging.
- Your doctor will examine your child for any other signs of puberty. Medical tests are usually not needed. If the doctor has any reason to suspect an abnormality, various tests may be performed as well, such as measuring hormone levels, x-rays to assess bone growth, or ultrasound scans of the uterus and ovaries.
- The doctor will continue to monitor your daughter to make sure there are no other signs of early puberty.
- If there is any reason to suspect a medical cause of early puberty, we will probably recommend a visit to an endocrinologist (a doctor specializing in the treatment of gland and hormone diseases). This specialist can perform tests and recommend treatments designed to interrupt the process of puberty, if necessary.
